As discussed in previous sections, a TeleMedicine Service must be able to share a communication link to exchange patient information and support remote treatment, care and advice. Clinical data and patient information can take many forms, but how it is captured can be viewed in three ways:
 
1. Indirect or direct capture: Indirect capture may be the reading of a recording (such as a patient vital sign) taken by using a stand-alone apparatus rather than an integral part of the TMS. An example of this may be the recording of patient blood sugar measurements taken within the home by the patient. This will involve the patient using a glucometer and then entering the data into a field on a preloaded form on the system. This is indirect data capture as there is no evidence of the reading being correct in terms of the actual measurements, the time or indeed to whom the blood sample belongs. If the patient was asked to show the taking of the sample over an Internet-based camera and then show the glucometer’s recording to the observer, then it could be seen to be direct.
 
2. Snap shot or trend: A single photo of a skin lesion is a snapshot, whereas a time series of the same is a trend. Does the remote diagnosis require a single piece of information or data captured over time? Confirmation of a snake bite may require a single shot of the region of envenomation, but the overall spread of the venom could be monitored through a sequence of further filming.
 

 
3. Live or delayed: Is there an urgency or response required that means the TMS should be captured or fed back in real time? The use of email to conduct a bereavement counselling would probably be a bad idea, but it would be appropriate for the transmission of STD samples via telepathology.
 
1. Look for three different scenarios in your workplace where a text-based solution could be used to ask a clinical question or to offer a diagnosis. In your answer for each, please state:
 
• The type of data that would be required for the process
 
• Available on site resources that could support or confirm the answer
 
• The software or communication medium that could be used
 
• If there is a publically accessible mechanism for obtaining the same result (e.g. could the patient find this data via another route themselves without much error?) Summarise these points for each scenario and write them up.
 
2. Find an example on the Internet of a recording of a patient ‘sound’. Play the file to assess:
 
1. Length of the recording
 
2. Quality of the recording (ie could a diagnosis be made or assisted by this?)
Look at the file size and the format.
 
3. Search the web for dermatological images. Submit one, along with details of the image resolution and your perception of its use for diagnosis. Try searching Dermatology Image Databases.
 
4. Look at the medical devices you use in your clinical practice and make a list of items which are manual and those which are electronic.
For the manual devices, establish whether there is an electronic version available.
 
For the electronic devices, check whether data can be directly captured or transferred to a computer system for each. If not, is there a device that will perform the task?
Where you can find a number of devices which will provide data for computer use, make a list of:
 
1. the device
2. which patient sign it monitors or captures
3. how it transfers data to a computer (e.g. data capture card, lead or modem)
4. how the data could be recalled and used (e.g. do you need a proprietary package or not?)
 
5. Consider and note possible content sources for core TMS guidelines development, within your specific practice and remote arena, covering the following elements:
 
• ‘Competency’ in managing the technical element of the system
• Clinical accountability for remotely prescribed care of the patient
• Gaining Informed Consent to using TeleMedicine
• Informing patients of their rights and responsibilities during a remote consultation
• Privacy consideration and maintaining confidentiality during and after the session
• Data integrity and network security
• Record keeping and record ownership
