Abstract
 

This project predicts the probable causes for a failure scenario
 

“new design software is installed, states that the installation was successful, and states that the software is operating properly. However, previously installed add-ons and other software do not work with this software and work on the new software is problematic, slow, and incomplete.”
 
I am showing causes of failure with the help of mind map tool and FMA&A report
Most of the software fails due to many reasons Inaccurate resources, use of immature technology, etc. These failures are mostly in development stage and easy to identify. But sometimeswell-developed software fails in some systems because of previous add-ons, system errors etc. These failures are very difficult to identify. In this project I came up with some possible causes for the failure of software in these scenarios.

Mind Mapping
Mind map is a graphical way to represent ideas and information.Information is structured in a way that resembles much more closely how brain actually works.
 
We create a central idea, add branches in the center we put our main idea and branches are subtopics.
 
It is an effective way of getting information in and out of our brain. It organizes the diagram
 
Brain Storming for Computer/Software Scenario
 

After lot of brain storming I came up with following questions
 

1. What could be the possible reason for the failure of software?
2. Does end user has any malware in his computer?
 

3. Which Operating System and which version he is using?
4. Did he upgrade the software or installed new?
5. Which version of add-ons he is using?
 

6. Are there any known bugs in add-ons?
7. What is the performance of new software in his system?
 

8. Which technologies are used for development of new software and existing addon/software?
9. Any warning messages during installation of new software?
 

10. Are these types of issues already faced by other users/ these type of issues mentioned in user manual?
 
Individual Research
 

In Brain Storming I came with many questions and researched for the solution for those questions. Studied many articles and blogs to find out why Software actually fails. Researched many topics and learned how software runs on Operating Systems, compatibilities of software, how software generally depends on other software, which components (environment variables, HKLM etc.) generally software uses etc.
 
After learning, I noted down many points for my document and started digging on some points that I felt needed for my document. Picked some reputed software and analyzed why they are not working in some systems and why they are performing slowly, studied those bugs and bugs report.
 
Then I started writing this document and came up with possible failure causes for software that installs successful but previously installed add-ons and other software do not work with this software and work on the new software is problematic, slow, and incomplete
 
Abstract
 

This work and brainstorming of cause describes many of the causes that a normal internet surfer comes across while they are directed to unwanted websites with undesirable clicks.
 
The theme is “Network/Server system: when we access the network, we alwaysget the wrong website. The sites we do get are not related to theones we are trying to access except for one thing: their names aresimilar”.
 
The mind map tool has been thoroughly used to bring out the easiness for getting this material readable and establish connections between various causes of the aforementioned problem.

Utilization of Mind Mapping tool
The mind mapping tool is a substantial approach towards the establishment of a graphical representation regarding an effect-cause relationship. It is in many places also called as the replacement for Ishikawa Diagram or Fish-Bone diagram. Here the Mind map is utilized by the connection of nodes to evaluate the reasons for redirecting the user on different websites with similar names of the user’s search option.
 
Brain Storming for the causes of Television Telecast Failure
This brain storming session is to redirect the complete focus over finding the major causes to locate the solutions. The agenda to focus is the redirecting of user’s account over distinct unnecessary website of almost similar name that the user enters in the web address.

1. The presence of adware in the user’s system can cause a huge impact over the web browsing facilities.
2. The presence of malware also is a reason for redirecting automatically, the user to some other website.
 
3. Not installing proper add-ons in the user’s browser can also add impetus to the redirection of user’s account on different websites.
4. Not updated antiviruses can lead to the success of malicious intents of many hackers and troublers over internet.

9. Attack by virus on the computer system.
10. Failure in the browser setting or if the browser is not updated.

An extensive research was conducted to bring out few of the high-end reasons for the problem presented. Apart from that some reasons are quite common and can be thought up by normal brain storming for the cause. These reasons are also extensively researched over internet to help the users finding solutions. The solutions are listed in the template down below in the section where the failure analysis is justified with assessment and assignment.
 
Abstract
 

This project acclaims the major issues that come during the operation of a television in home or offices.
 
The theme is “Television: the system is hooked to cable; it is on and receives some channels just fine, but other channels are not available. At other times, those channels are available. All channels should be received all the time, but intermittently they are not.”
 
By using the Mind mapping tool, the major issues regarding the aforementioned scenario has been highlighted. The major causes of television channel telecast failure can be sufficed into network issues, television feature issues and other server issues. Due to many reasons and lack of technicality, a normal person is unable to find these attributes and suffer by the hands of fate to call an expensive electrician or television technician.

This whole failure analysis has been diagrammatically featured in a mind map. A mind map is more of an expanded and branched version of a diagram which is utilized to give tributary functional points to a major agenda. For example, one major agenda is kept in the middle and many connections through wires are made to different sources connected with the agenda. It also helps to make a clear understanding from presenter’s perspective.
 
Brain Storming for the causes of Television Telecast Failure
Many sources over internet and other books were referred before applying any kind of brainstorming session. Still, after some poised efforts, the following reasons felt to make up for the list of reasons a channel in the television telecasts itself intermittently and not always.
 

1. The very first reason that comes to anyone’s mind is the server problem. And it is true too. Sometimes because of bad server of the channel from its headquarters restricts the telecast of the channel.
2. The next reason can be the change in settings of the television. Sometimes while people do not maintain the child lock settings and keep it off, the children in the house, oblivious to the functioning press some buttons over the remote that can disable the visibility of the channel.
 

3. Another reason that suffices the cause is the change in weather. Sometimes the weather is too cloudy and windy that restricts the radio waves signals carrying transmission of channels to reach the channels to the source.
4. One more reason can be the presence of coaxial network in the house or some rooms. The disintegration of the coax cables of the terrestrial disc and the coax network wires can cause discrepancies in the functioning of channels.
 

5. The extra devices connected from the television like the DVDs and VCR players can also create the impact of channel invisibility.
6. Loose connections from the coaxial cables coming out of the wall to connect the to the television.
7. Some televisions have auto program features in them. That feature must be kept on all the time, otherwise the discrepancies in channel transmission can be observed.
 

8. Hidden channels from the television settings also create confusions for the user and makes them dubious about why the channels are not telecasting.
9. Change in the frequency of transponders. Some are strong causing the telecast and some are not.

The carried research holds a really high importance to finish this task. Apart from brain storming in the questions, the answers are too necessary to be found and addressed.
 

For answering the above brain stormed reasons for the non-telecast of some of the channels in the television, many sources were referred from internet websites and information portals. After combining them and taking keep scrutiny to their advised solutions, a compilation of self-researched solutions is presented down below in the further sections.
